Home from Hospital: Lewes & Wealden

• Voluntary service, providing support and practical help to older people in the Lewes & Wealden areas, for up to six weeks after discharge from hospital.
• Service available to anyone, over the age of 55, living in Lewes or Wealden, who is about to be discharged from hospital, or has just been discharged.. Volunteers may be able to visit client at home, for up to eight weeks, after discharge, giving practical and emotional support.
