Dementia Support Service East Sussex

The Dementia Support Service is provided by the Carers Breaks and Engagement Team at East Sussex County Council.

The Dementia Support Service is available to support people living with dementia in the community and their families and carers in the Hastings and Rother, Eastbourne, Hailsham and Seaford, High Weald, Lewes & Havens areas of East Sussex.

The Dementia Support Service will work with you, and your family and friends to:

• explore with you appropriate support and well-being opportunities
• identify and access any support you may need, for example, attending social groups or clubs, healthcare appointments or any other activities that are important to you
• work with other agencies to coordinate any care and support you or your carer may need
• develop a relationship with you and any family members or friends to begin careful and considerate discussions on planning for your future.

How do I get in touch with the service?

Contact Health and Social Care Connect on 0345 60 80 191 or apply online at adultsocialcare.eastsussex.gov.uk and ask to be referred into the service.

We will call you or your representative to arrange to visit you at your home, or at a venue that suits you.
